The application
for a contribution can be submitted in the Moje VZP application or by post
until 15 December 2023. If you submit your application in person at a
branch, in a collection box or mailbox of the branch or at the post office, the
deadline is 31 December 2023. The application must be accompanied by a proof of
payment for the aid or service issued from 1.10.2022. When submitting an
application for a contribution for the first time or when submitting an
application after a change of health insurance company and re-registering with
VZP, you must also submit a proof of diagnosis.

The specific diagnoses covered by these contributions are listed on the VZP website.
The application for the contribution is submitted 2 times a year, namely:
- in the first half of the year from 1 January 2023 to 30 June 2023 for the first half of the amount;
- in the second half of the year from 1 July 2023 to 31 December 2023 for the second half of the amount. If submitted by post or in Moje VZP, the deadline is 15 December 2023.
When you apply for a contribution for the first time, or when you apply after a change of health insurance company and re-register with the VZP, you will also need to attach proof of diagnosis to your application.

First,
join the programme by submitting an application for a voucher at any VZP branch
or at opatrujse@vzp.cz with your name, surname, insurance number and the
e-mail address to which the voucher should be sent. You will then select a
therapist from the list on the VZP website. The application for a contribution
can be submitted in the Moje VZP application or by post until 15 December
2023. If the application is submitted in person at the branch, in the
collection box or mailbox of the branch or at the post office, the deadline is
31 December 2023. The application must be accompanied by a proof of payment
for the therapy (with the date of payment after the voucher is issued, but no
later than 31 December 2023) and an online "Satisfaction Survey" must
be filled in after the last completed therapy.
