* a contribution of up to 1 500 CZK is available for these screening examinations
You can apply for a contribution in the Moje VZP app or by post until 15 December 2023. If you apply in person at a branch, in a collection box or mailbox of the branch or at the mailroom, the deadline is 31 December 2023. You must attach proof of payment for the activity, goods or services paid for from 1 October 2022 and either a pregnancy certificate or the child's birth certificate. Within one pregnancy (during pregnancy up to 12 months after the birth), a maximum of CZK 3,500 can be drawn even if a partial or maximum contribution of CZK 3,000 has already been used in 2022.

The application for the contribution can be submitted in the Moje VZP application or by post until 15 December 2023. If the application is submitted in person at the branch, in the collection box or mailbox of the branch or at the mailroom, the deadline is 31 December 2023. The application must be accompanied by a proof of payment for baby supplies or aids at the pharmacy paid from 1 October 2022 and the child's birth certificate if it was not already submitted when registering the new-born with VZP.

Attach to the application a proof of payment for baby formula or breast milk from the bank or rehabilitation care paid from 1 October 2022 and the child's birth certificate together with a document confirming the child's birth weight (e.g., discharge report from the maternity hospital).
