For active donors of blood, plasma, platelets or bone marrow

for spa-type rehabilitation/recovery activities or activities provided by a physiotherapist, or for health exercises	18–65 years	2 000 CZK*
for dental hygiene or a dental hygiene package	18–65 years	1 000 CZK**

* Upon presentation of confirmation of at least one donation per calendar year

** upon presentation of confirmation of at least two donations per calendar year

The application for a contribution can be submitted in the Moje VZP application or by post until 15 December 2023. If the application is submitted in person at the branch, in the collection box or mailbox of the branch or at the mailroom, the deadline is 31 December 2023. The application must be accompanied by proof of payment for the activity, goods or services paid for from 1 October 2022, together with confirmation of the free donation of blood, plasma, platelets or bone marrow.

For non-active blood donors

The application for a contribution can be submitted in the Moje VZP app or by mail by 15 December 2023. If the application is submitted in person at a branch, in a branch collection box or mailbox, or at a mailroom, the deadline is 31 December 2023. The application must be accompanied by proof of payment for the activity or service paid for from October 1, 2022, along with a commemorative award certificate.

For organ donors

contribution	age	amount
for spa-type rehabilitation/recovery activities, the purchase of sports equipment or vitamins, food supplements and medical devices	18+	10 000 CZK

The application for a contribution can be submitted in the Moje VZP application or by post until 15 December 2023. If the application is submitted in person at the branch, in the collection box or mailbox of the branch or at the mailroom, the deadline is 31 December 2023.
The application must be accompanied by proof of payment for the activity, goods or service paid for from 1 October 2022 along with a personal letter from the Director of the VZP.
